1.Applications and forms

Safety and health representatives may use this form to register with WorkSafe to receive relevant information about occupational safety and health and events such as seminars. Registered safety and health representatives receive a CD with codes of practice and guidance notes, a safety and health representative identity badge, the handbook, a subscription to SafetyLine magazine and email updates.

This form is to be used by the person who conducted the election and who is responsible for notifying the WorkSafe Western Australia Commissioner and the employer of the results of the election.

From April 4 2005, qualified safety and health representatives may, under Part 6 of the Occupational Safety and Health Act, issue provisional improvement notices (PIN). This form may be used to issue a PIN.

This form may be used to request the review of a provisional improvement notice issued by a qualified safety and health representative.

3.Support 

Community education officers

Safety and health representatives are offered support through WorkSafe's two community education officers.

The community education officers encourage people to become safety and health representatives, and provide the assistance with their direct queries: email, shreps@commerce.wa.gov.au.

The community education officers are available to come to your workplace to give talks on the role of WorkSafe, to explain the obligations of employers and employees under the act, and to promote the role of safety and health representatives.
