Accident and incident reporting
Provides information on reporting a dangerous goods or mining accident / incident, including links to forms; guidelines; and procedures.
Dangerous goods
Section 9 of the Dangerous Goods Safety Act 2004 (WA) requires certain dangerous goods incidents (called reportable situations) to be reported to Resources Safety as soon as practicable.

Mining
Under the Mines Safety and Inspection Act 1994 (WA), there are specific reporting requirements for:
- accidents involving injury to persons
- occurrences (commonly referred to as incidents).
The data collected from the report forms are also entered in Resources Safety’s AXTAT and incident reports databases. The incident reports database is used to record and retrieve information about incidents in the workplace, and provides a useful tool for identifying trends in reported incidents and assessing risk.
